Step 2. Open the downloaded .pdf survey in Adobe Acrobat Reader 6.0 or higher and complete the required fields.

WHY CAN'T I SAVE FORMS COMPLETED IN ACROBAT?
Adobe Reader does not allow you to save completed forms. It only lets you print them. You must print your form before you quit the application. Otherwise, the data you entered will be lost. The only way to save a digital copy is to use: Acrobat Approval, Acrobat Standard, or Acrobat Professional.
